Alta Health Services is excited to announce a full time Physical Therapist position for our home health division in beautiful Salt Lake City, UT. Our PT will enjoy a generous hourly rate, a $58-$70 per visit rate, and a $10 driving bonus. We pride ourselves in the quality of care we provide, and we offer care options to suit every need.

We're excited to offer our therapists an opportunity to deliver patient centered care. We empower therapists to decide what their patients need, and allow enough time in their day to deliver. There are no 15 minute encounters, we do not schedule at 125% production, and we encourage our therapists to spend the time to ensure each patient receives the care they require.

Our Physical Therapist will help our patients with the following:
* Gather background data, review referrals, select and modify evaluation tools, and evaluate patients determining the level of our patient's physical and psychosocial functioning.
* Develop and revise the plan of care in consultation with the physician and other care-team members.
* Responsible for planning, evaluation, and treatment implementation of assigned caseload.
* Preparing clinical notes.
* Evaluating outcomes.
* Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment.

Pay: $40.00 - $47.00 per hour
